Practicum Log

Purpose

This assignment is designed to help students document for all of the activities they engage in to meet the objective and course outcomes outlined in the learning agreement and to complete the practicum work. The Practicum Log reflects all of the time and activities the student spends completing the practicum requirements. It identifies the nursing informatics (NI) practicum activities and tracks a student’s hourly practicum hours, helping a student realize where time has been spent and what has been accomplished.

Through this assignment, the student will demonstrate the ability to

CO 1: Apply leadership skills concepts of project management as an informatics nurse specialist in an organizational setting to develop, implement and evaluate successful project plans. (PO 4,5)

Requirements

Complete the weekly Practicum Log using the form provided; submit signed original log each week into Drop Box; “0” points if log not signed and dated by mentor.
Maintain the weekly log throughout the course.
Notify the professor and/or mentor of any concerns during the completion of the log.
