Good afternoon, Dr. .... I am calling you today because you are a Mental Health Specialist and I need to refer my patient to you for evaluation of PTSD including treatment recommendations.
The patient is a 53-year-old male Army combat veteran who presented one month ago because he has been struggling with increased anxiety, anger, and depression x several years. His wife accompanied him to the visit and stated that he does not socialize outside from the home engages in ETOH almost daily, has lost the desire to participate in any hobbies, and has recently become physically aggressive while sleeping. The patent stated he feels emotionally numb, has trouble sleeping, and that he is an “evil person” for the things he has done in the past.
