C.B. is a 70-year-old Caucasian woman with a history of hypertension, and 26-year 1PPD smoking history, who presents today with complaints of trouble breathing for several years but has progressively gotten worse. Her shortness of breath is worse with exertion and improves with rest. She also complains of some swelling in her feet and ankles, and fatigue. Upon exam, patient has a pansystolic murmur, and bilateral 1+ pretibial edema, lungs are clear to auscultation. The echocardiogram showed mean gradient consistent with severe aortic stenosis, estimated area consistent with severe/critical aortic stenosis 0.65cm2.
Medications/Treatment: Depending on if the patient was in agreement to get a valve replacement, that would determine the patient’s medical treatment. (Otto et al., 2020).I would also start the patient on Rosuvastatin 5mg.According to the guidelines, patients with calcific AS, statin therapy is indicated. (Otto et al., 2020). This patient also has poorly controlled blood pressure with the amlodipine that she is already on. I would add an ACE inhibitor for her blood pressure. I would start her on Ramipril 2.5mg daily. I would also add on an aldosterone antagonist, that has shown to be beneficial in treating hypertension as well as heart failure. (…). I would prescribe her Spironolactone 12.5mg PO Daily………..
