During history taking, one must ask the symptoms, risk factors, past medical and family history, medications, etc. to be able to narrow down differential diagnoses since it can include many disorders such as obstructive, parenchymal, cardiac or compensatory. Heart disease and lung disease are the most common causes of dyspnea such as heart failure, pneumonia, obstructive lung disease (asthma, COPD), and pulmonary embolism.
Since the differential diagnosis of dyspnea can be broad, we can narrow it down to the four most common causes of dyspnea:
- Heart failure (HF)- Focus on the patient's signs and symptoms, risk factors, for example, a past history of myocardial infarction is a risk factor for HF. Physical examination like jugular venous distention, crackles, peripheral edema are also suggestive of HF.
