Case Study: Allergic Asthma
A: Primary Diagnosis-Allergic Asthma (baker’s asthma)
My primary diagnosis for Michele is allergic asthma. This condition can be classified under “Unspecified asthma with (acute) exacerbation” (J45.901) in ICD-10 code. This diagnosis was informed by the symptoms exacerbated by the patient at work, exam results, and recent history. Michelle reports experiencing shortness of breath only when at the workplace where she bakes bread/pastries. She also reports that she started experiencing this problem when she started working at the bakery. According to Luo (2018) and DerSarkissian (2019), allergic asthma is a condition that leads to the contraction of the airways because of the overreaction of the immune system to allergens, which find their way into the extra-sensitive airways.
Other than constriction of airway smooth muscles, the overreaction of the immune system to allergens triggers the production of mucus, sneezing, itching of eyes, and running of the nose (DerSarkissian, 2019). The constriction of smooth airway muscles leads to shortness of breath (which is reported by Michelle), wheezing, coughing, feeling tightness in the chest, and quick breathing (DerSarkissian, 2019; Luo, 2018; Hall, 2019). According to Luo (2019), allergens such as eggs, wheat, peanuts, tree nuts, smoke, strongly scented perfumes, and fumes can trigger allergic asthma. Thus, since Michelle works in pastry, her contact with wheat flour and smoke could be the primary triggers of this condition. She is also under Zyrtec, a medication that is used in reducing the severity of allergic response, hence indicating that she has an allergy (Nall, 2019)....... Discussion Continue
