Problem statement:
AC is a 17 y/o female high school student who presents to the clinic with a two-month history of fatigue/tiredness. Per report, she admits to being frustrated, irritable, anhedonia, tearful, emotional lability, decline in school performance, and unintentional 25-30 weight gain. She admits to being less active since quitting the track team.
She states her current diet consists of pizza and french fries. Physical exam is noted for obesity, abdominal and buttock striae, acanthosis nigricans and mild comedonal acne on forehead. She denies thoughts of hurting self or others. She admits to social alcohol use and past marijuana use with ex-boyfriend. Denies sexual activity at the present due to recent breakup with her boyfriend.
