Within the state of Illinois there are four different types of advance directives the Durable Power of Attorney for Healthcare, the Living Will, the Declaration for Mental Health Treatment Preferences, and the Practitioner Orders for Life Sustaining Treatment (POLST) (Nurse Practice Act, 2021). The first three documents can be discussed with a healthcare professional, an attorney, or a physician. The Power of Attorney paperwork, Living Will, and Mental Health Treatment Preferences can be located online and does not need to be filled out with an attorney nor a physician. The physician may not witness a Mental Health Treatment Preference. Within the state of Illinois, the APN can sign a POLST form with a patient, along with another witness.
