History of Present illness: 72 y/o Male presented with Shortness of breath, fatigue, and swelling in abdomen and b/l LE.
History of present Illness: a 72 year old male, with PMH of hypertension, CAD, hyperlipidemia, coronary stent placement 6 months ago. Former smoker, quit 6 months ago. Patient presented with complaints of shortness of breath, worse with physical activity and when lying down, fatigue, edema in b/l LE and abdomen, cough with white frothy mucus. Patient denies abdominal pain, chest pain, GU or GI complaints.
Past Medical History: HTN, CAD, hyperlipidemia.
Hospitalizations/Surgeries: coronary stent placement 6 months ago.
Medications: takes metoprolol, ramipril, clopidogrel, atorvastatin, ASA, Tylenol prn.
Allergies: NKDA.
Preventive health: took flu shot, pneumonia shots.
Family history: Father and mother deceased from heart problems.
Social history: married and lives with his family. Former smoker. Drinks 1-2 drinks of etoh a day.
Review of systems:
General: shortness of breath, fatigue, worsening of abdominal and leg swelling, not sleeping good fur to orthopnea and discomfort.
Integumentary/Breast: b/l lower extremities swelling. Skin is pink, warm, and dry. Normal skin turgor, no rash, lesion.
HEENT/Neck: denies neck swelling, neck pain, denies double vision, or visual changes. Denies history of cervical lymphadenopathy.
Cardiovascular: history of coronary artery stent 6 months ago. Complains of b/l LE and abdomen swelling, decreased exercise tolerance. Denies chest pain, chest pain or discomfort with exertion, denies palpitations……….. Continue
