You will use this document for EACH of the clinical courses, with the objective to complete as many of the competencies as possible by the end of the program. It is recommended that you share the competency checklist with your preceptor at the start of each rotation so that efforts can be made to see the types of diseases & procedures that you are missing. There is no set number of each that needs to be obtained.
Diagnosis-Mgmt Competency Tab
For each competency, mark a date in the column to indicate when the competencies were met for each condition/type of visit.
Procedure Competency Tab
For each procedure, mark the date in the column separately for the procedures observed and assisted. Procedures mark with (*) are ONLY to be observed
