1. Primary Diagnosis
The primary diagnosis for the patient at this time would be hypothyroidism. Pertinent positive findings for this patient include: fatigue, female gender, depression…….
2. ICD Code
3. Medication
4. Patient education
The patient should take the medication at night to avoid night time insomnia and the medication should be taken on an empty stomach to increase absorption (Dunphy, Winland-Brown, Porter & Thomas, 2019)…….
5. Additional Testing:
I do not believe any additional testing needs to be done at this time. The patient should have their TSH levels monitored……
6. Referral
No Referral is needed for hypothyroidism…..
7. Provide an active problem list for this patient based on the information given in the case
8. Changes to the patient’s overall treatment plan
The only change I would make at this time for the patients overall treatment plan is to start the medication….
9. Follow up
Follow up plan is to recheck her TSH levels in 6 weeks to check for medication effectiveness…..
