Hello, my name is ..... and I give my permission for this examination to be recorded and submitted to Chamberlain College of Nursing for grading purposes
Now we will start the exam after I do the 360 view.
HEAD
I will begin with inspecting your face, and I see no discolorations or any lesions. His head is midline and symmetrical.
Next, I will palpate his lymph nodes, and I will begin with the preauricular lymph nodes, in front of his ear, and postauricular, behind his ear, his occipital lymph nodes, in the back of his head and move palpate his tonsillar lymph nodes, under his jawline, and submandibular lymph nodes, and then his submental. Now I will palpate his anterior cervical lymph nodes, and his posterior cervical lymph nodes, ending with his supraclavicular lymph nodes, right about the clavicles. I did not feel any enlargement, and they are equal bilaterally.
Cranial Nerve V
Next, I will test his trigeminal nerve, which is cranial nerve number five. It has both a motor and sensory component. For the motor portion I will have him clench his jaw while I palpate over the masseter muscle. I don't feel any distortions, and he has great strength. For the sensory portion I will have him close his eyes, and have him let me know where he feels my touch. Forehead, right cheek, left cheek, chin, nose.
Cranial Nerve VII
Now I will test his facial nerve, which is cranial nerve number seven. Ary please smile, frown, raise your eyebrows, puff your cheeks, and pucker your lips. All equal bilaterally................. Continue
