Hello, my name is , and I give my permission for this examination to be recorded and submitted to Chamberlain College of Nursing for grading purposes

 

Now we will start the exam after I do the 360 views.

 

HEAD

I will begin with inspecting your face, and I see no discolorations or any lesions. His head is midline and symmetrical. Next, I will palpate his lymph nodes, and I will begin with the preauricular lymph nodes, in front of his ear, and postauricular, behind his, moving to his occipital lymph nodes, in the back of his head and then move to palpate his tonsillar lymph nodes, under his jawline, down to the submandibular lymph nodes, and then his submental. Now I will palpate his anterior cervical lymph nodes, and his posterior cervical lymph nodes, ending with his supraclavicular lymph nodes, right about the clavicles. I did not feel any enlargement, and they are equal bilaterally.

 

Cranial Nerve V 5

Next, I will test his cranial nerve number five, which is the trigeminal nerve. It has both a motor and sensory component. For the motor portion I will have him clench his jaw while I palpate over the masseter muscle. I don't feel any distortions, and he has great strength. For the sensory portion I will have him close his eyes and have him let me know where he feels my touch. (Forehead, right cheek, left cheek, chin, nose).

 

Cranial Nerve VII 7

Now I will test his cranial nerve number seven, which is facial nerve. Jeremy, please smile, frown, raise your eyebrows, puff your cheeks, and pucker your lips. All equal bilaterally.

 

EARS

Now, I'm going to inspect Jeremy’s ears for any nodules or any skin lesions. His ears look symmetric with no signs of nodules or lesions. I’m going to assess his ear canal with the otoscope. I’m pulling his ear up and back and I see his external auditory canal is clear, and has no swelling, redness, drainage, or cerumen. The tympanic membrane is pearly gray, and there is no effusion in the middle ear. I will repeat on his left ear, I’m pulling his ear up and back and I see his external auditory canal is clear, with no swelling, redness, drainage, or cerumen. Now I'm going to palpate his pinna, for any nodules or tenderness. Are you having any tenderness, Jeremy? No. And now his tragus, in front of the ear, are you having any tenderness? No.

 

 

Cranial Nerve VIII 8

Now I will assess his/her cranial nerve number eight, which is acoustic nerve, and that is his hearing. I’m going to have Jeremy cover one of his ears and I’m going to whisper three words, please repeat those three words (One, two, three).
