Rebecca is a 23 year-old female law student with a history of infectious mononucleosis at age 15, presenting today with chief complaint of acute onset fever and sore throat x2 days. Associated symptoms include a mild headache, chills, and bilateral swollen neck glands. She reports that her roommate was also sick earlier this week, denies obtaining annual flu vaccination but did obtain COVID series. She has been taking acetaminophen for fever and eating ice cream to alleviate sore throat. She describes the sore throat like an "ice pick" with 10/10 pain resulting in difficulty swallowing. Physical exam reveals fever of 101.5, erythematous posterior pharynx, enlarged tonsils (right slightly larger than the left), prominent exudate, halitosis, and bilateral tender cervical lymphadenopathy but is negative for adventitious breath sounds.
