Case Study

Barry is a 65-year-old male who presents for his yearly physical examination and follow up from Holter monitor. His review of systems is significant for fatigue, complains of feeling “heart is racing”, frequent urinating, and unable to do daily exercises with out feeling short of breath. His previous medical history is notable for hypertension and overweight.  He reports his understanding that he should be on a low fat, 2gram , and no added salt diet with moderate aerobic exercises 4-5 times per week but also notes that still running a business full-time makes this difficult to achieve.


He has NKDA and takes a multi-vitamin and Norvasc 5mg daily. Barry reports a family history of Hypertension (HTN), and Coronary Artery Disease (CAD) (Father). He is a nonsmoker. Occasionally consumes ETOH. His diet largely consists of red meat. He drinks water or milk with every meal , 1-2 cups of coffee in the AM and an additional 1-2  cans of soda per day. Previous labs and exam last year are unremarkable.


Vitals today: BP 129/80 mm Hg, pulse 102, temperature 98.5, respirations 20, height 5’11”, weight 185  pounds. Urine dip negative, fasting plasma glucose 110mg/dl, HgbA1C is 5.9, TSH 2.40mU/L and Free T4 0.9 ng/dl.  CBC WNL,EKG shows irregular QRS complex no visible P-wave, ventricular rate 100. Holter monitor shows irregular heart beats at times. The physical exam is notable for non-valvular atrial fibrillation.
