My hospital has a problem with COPD readmissions. We are currently at 23% compared to the national average (19.6%), according to Hospital Compare. According to research, the use of follow up telephone calls can help reduce rehospitalization. Currently we do not call patients, so we need to initiate this nursing practice issue of follow up phone calls. (Leppin et al, 2014).

Briefly describe your rationale for your topic selection. Include the scope of the issue/problem.

COPD is a serious disease in the U.S. The 5-year mortality rate for people with COPD typically ranges from 40% to 70%. COPD is part of Medicare's Hospital Readmissions Reduction Program (HRRP), which penalizes hospitals for excess 30-day, all-cause readmissions after a hospitalization for an acute exacerbation of COPD (CMS, 2018) These readmissions cost more than $26 billion a year (Leppin et al. 2014.
