Harvey Hoya is a 57 year old male construction worker who presents with chief complaint of high blood pressure found at a health fair, and has headaches in the morning with increased fatigue and 10 lb weight gain the last 6 months. Positive snoring while sleeping and difficulty sleeping. Positive family history of stroke, MI, and high blood pressure diagnosis. Patient states to smoke a pack of cigarettes a day since he was in his 20's, daily coffee intake, daily fast food intake, and weekly alcohol intake. Limited daily exercise. Physical exam reveals severely elevated blood pressure, BMI of 28.7, waist of 45 inches. No SOB or chest pain. No known allergies and overdue for annual physical exam.
