Stats that this is the worst headache that feels like throbbing; starts from the left temple and radiates to her whole head. Rating her pain 8 out of 10, associated with nausea, photophobia, and phonophobia. Tylenol didn't help much. Laying down in a quiet dark room helped to alleviate symptoms. She reports a recent increase in stress and a decrease in sleep due to a recent breakup with her boyfriend and school workload. Positive for past history of headaches like this, which now occurs every four to six weeks. Mother and sister have similar kinds of headaches. All cranial nerves are grossly intact. Denies any chest pain, seizure, shortness of breath, or any visual disturbance.
